Dear Patient:
Thank you for contacting our office to schedule an appointment
with one of our providers. Our goal is to help make your
visit with us as easy and pleasant as possible.
One way to make things easier for you is to have you complete
the following Patient
Information Form at least two weeks prior to your
visit. This form is for the demographic and insurance information. Completing this
Patient Information
Form prior to your appointment will
allow us to have everything prepared on the day of your
visit and will avoid any possible delays.
Please also bring your insurance card and driver’s license
with you to your appointment. If you have an HMO, or Point
of Service plan, please be sure to call your primary care
physician (PCP) to obtain a referral for your appointment
with us. This, too, will help avoid any delays.
